MEMORANDUM RECEIPT
The equipment described below is being issued to the recipient for the purpose of the project and subject to IDEALS policy and conditions on the use of equipment.
Condition of Use:
- The above described equipment is/are property of IDEALS, Inc. and to be used exclusively for the project. The equipment is to be returned to IDEALS at the end of the project or at the end of contract of the recipient.
- The recipient takes full responsibility of the equipment. The recipient should use the equipment with utmost care. Any problem/defects and/or loss arising due to negligence or misuse shall be to the recipient’s account.
By signing below, the recipient hereby acknowledge agreement to the above noted condition of use and of receiving the above listed equipment in good order and condition.
